Home and Community Support Program Overview
Continuum of Colorado’s Home and Community Support Program is a licensed Home Care Agency established in 2012 providing quality services to adults and children by trustworthy and caring providers. We collaborate with the individual and families to develop a comprehensive, person-centered care plan which allows the individual to be an active participant. Families and individuals are encouraged to choose their own providers and manage the direction of the services.
Continuum of Colorado’s Home and Community Support Program is dedicated to employing the best providers for individuals, regularly utilizing the Family Caregiver Act to hire family members and family referred applicants to provide the needed services. All employees complete required trainings specific to the individuals’ needs.
We offer a range of services:
Support with grocery shopping, appointment assistance, general money management skills, and completing Activities of Daily Living (ADLs)
Supervision for the individual to give relief to the primary caregiver(s)
Supported Community Connections:
Support with developing skills to make connections within the individual’s community
Homemaker (Basic and Enhanced):
General housekeeping tasks in the individual’s primary residence, including cooking, cleaning, and laundry
Support with developing self-advocacy skills
Physician referred services performed by a licensed massage therapist
Catered hot lunch and snacks offered daily
